چکیده

T he presence of thrombus in the left atrium (LA) is an important factor in the selection of cases for percutaneous mitral balloon valvotomy (MBV). This is because of the danger of embolisation caused by manipulation of guidewires and balloon catheters. The American College of Cardiology/American Heart Association recommendations for MBV emphasise absence of LA thrombus. Transoesophageal echocardiography (TOE) has a high sensitivity for detection of cavity and appendage LA thrombi (93–100%) and is normally carried out before MBV. We report our experience of MBV in patients who had an LA thrombus at initial TOE.

Blooming Flower in Misty Left Atrium

Left atrial (LA) thrombus is known as a common complication of rheumatic mitral stenosis. It is more common in the presence of atrial fibrillation (AF). Incidence of LA clot formation in sinus rhythm is 2.4–13.5% and the incidence is as high as 33% in patients with mitral stenosis in AF. Because transthoracic echocardiography may not detect the left atrial appendage (LAA) thrombus, transesophag...

Tsung O. Cheng, MD, Professor of Medicine, George Washington University, Washington, D.C. (USA) I read with interest the article on the use of transesophageal echo-cardiography (TEE) in monitoring emergency percutaneous balloon mitral valvuloplasty (PBMV) in a patient with massive left atrial thrombosis by Chirillo et al. [1]. Although I agree that TEE was of great value in monitoring each step...

An unusual complication of cardiac catheterisation during BMV.

1 of 3 DESCRIPTION Rheumatic heart disease is the most common cause of mitral stenosis, and percutaneous balloon mitral valvotomy (BMV) has stood the test of time as the standard therapy for the same. We describe a very unusual complication of cardiac catheterisation that occurred during an otherwise uneventful procedure. A 65-year-old woman was diagnosed with severe mitral stenosis due to rheu...
